Non-profit comprehensivehealthcare organization is searching for a Community Nurse Practitioner (CNPS) whowill work in collaboration with contracted community primary care physicians(CPCP) to ensure proper oversight and coordination of participant care with theInterdisciplinary Team. The CNPS will work with the community physician tocoordinate and support the participant's functional, clinical, and psychosocialneeds, with an emphasis on independence and wellness. This role will work asthe liaison between the participant, the community physician, and plan staff todevelop and implement the plan of care, including person-center goaldevelopment, communicating changes in participant condition, and facilitatingprimary care preventative services.

This is a 5 day per week assignment. Monday -Friday 8:30 - 5:00 - No weekends - No evenings

Compensation: $80.00 Per Hour

Education:

• Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.
• Minimum of two years in clinical nursing practice in-home care, care management, or related field required.
• Licensed to practice as a Nurse Practitioner in NY required
• DEA Certification required
• AANP or ANCC Board Certification required
• Spanish or Bengali speaking preferred
